CCC IDENTIFICATION CERTIFICATE

With an identification certificate from ChatsCanadaCats, your cat will be registered in the CCC registry. The identity of your cat will be protected. Your cat will be identifiable if lost or stolen.

In order to obtain a certificate of identification, your domestic cat must be neutered and microchipped.

How do you get your cat microchipped? Consult your veterinarian or attend one of our microchip implantation clinics.

Procedures for sending forms:

2. Print, complete and sign the form.
3. Send the form by mail to ChatsCanadaCats - Succursale Bureau Chef, Case postale 15, Laplaine, Terrebonne, QC, Canada J7M 1Z2) or email to infos@chatscanadacats.ca

Procedures for sending payment:

1. Calculate your total including taxes applicable to your province (instruction on the form).
2. Send a personal check to ChatsCanadaCats with your form by mail.
